That is a difficult question to answer, because if a candidate is too honest or finds it easy to think of some very bad decisions he has made, even if he has learnt from them, depending on how bad or damaging the decisions were, I would still question his intelligence and wonder whether he is the right candidate or whether he is likely to make some more very bad, albeit different decisions and learn at the expense of the company.
Ideally you would hope that the candidate will recall situations where in hindsight he could have come to a better decision, ....
